EOFError Python

I have a widget with some dropdown options. Based on the user's input I have to ask for followup information. I am seeing an EOFError for my input statement for card_info in the follow_up function. The function is not taking user input for the card info either. I tried putting it in a try/catch block but it didn't help. How can I get the function to take card_info from the user so I can use that information to call other functions?
